Integra here has had enough of this dancing around nonsense, and is so upset she is accidentally drawing blood! She's supposed to look like she's both about to cry and about to shoot him. For Alucard's part, well, she's never been quite so forward. Also, her neck, chest, and sweet virgin blood are all exposed. So he's kind of both stunned and vaguely aroused.
